Welcome to the Paxlovid Patient Data Collection System (the “System”). This site is for purpose of gathering, monitoring, and documenting adverse events in connection with or resulting from the use of Nirmatrelvir/Ritonavir (Paxlovid), which specifically includes but is not limited to the reporting of information submitted or provided through the System, including information on adverse events themselves, to relevant government offices in compliance with Pfizer’s adverse events and / or safety reporting and / or monitoring obligations whether in or out of the Philippines, and in accordance with internal Pfizer policies relative to adverse events and / or safety monitoring; to sharing reported adverse events with vendors, providers, advisers, and affiliates of Pfizer for purposes of storing, monitoring, recording, and of documenting adverse events where in or out of the Philippines.

- Use of this System. This site is for purpose of gathering, monitoring, and documenting adverse events in connection with or resulting from the use of Nirmatrelvir/Ritonavir (Paxlovid), which specifically includes but is not limited to the reporting of adverse events to relevant government offices in compliance with Pfizer’s adverse reporting obligations whether in or out of the Philippines; to sharing reported adverse events with vendors, providers, advisers, and affiliates of Pfizer for purposes of storing, monitoring, and of documenting adverse events where in or out of the Philippines. Certain sections of this System are intended for particular audiences including Pfizer's employees, customers and shareholders, as well as members of the health care community, including but not limited to physicians, pharmacists, pharmacist assistants, and the like, and the general public. Your access to and use of some portions of this System, as well as the information contained in this System, may be limited or restricted, as may be necessary to comply with applicable laws and regulation, and is subject to this Terms of Use Agreement. By accessing and using this System, you accept, without limitation or qualification, this Terms of Use Agreement.
- Content. As may be applicable, Pfizer will use reasonable efforts to include accurate and up-to-date information on this System but makes no warranties or representations of any kind as to its accuracy, currency or completeness. You agree that access to and use of this System and the content thereof is at your own risk. Pfizer disclaims all warranties, express or implied, including warranties of merchantability or fitness for a particular purpose. Neither Pfizer nor any party involved in creating, producing or delivering this System shall be liable for any damages, including without limitation, direct, incidental, consequential, indirect or punitive damages, arising out of access to, use of or inability to use this System, or any errors or omissions in the content thereof. This limitation includes damages to, or for any viruses that infect, your computer equipment.
- Indemnification. You agree to indemnify, defend and hold harmless Pfizer, its officers, directors, employees, agents, suppliers and third party partners from and against all losses, expenses, damages and costs, including reasonable attorneys' fees, resulting from any violation by you of these Terms of Use, particularly including but not limited to any misrepresentation, and / or making of any false, misleading, and / or incorrect statements that affect or otherwise impact the veracity, reliability, and / or credibility of any report on adverse events.
- Privacy. Pfizer respects the privacy of its System users. Please refer to Pfizer’s Privacy Policy that explains users' rights and responsibilities with respect to information that is disclosed on this System.
- Third Party Websites and Links. This System may contain references or links to other websites, which are maintained by third parties over whom Pfizer has no control. Such links are provided merely as a convenience. Your use of any such website or platform shall be governed by the Terms of Use provided therein.

- Governing Laws. This Terms of Use Agreement and your use of the System shall be governed by the laws of the Philippines without regard to its conflicts of laws principles. Any legal action or proceeding related to this System shall be brought exclusively in a federal or state court of competent jurisdiction sitting in Makati City, Philippines.
- Miscellaneous. If any provision of this Agreement is held to be unlawful, void or unenforceable, then such provision shall be severable without affecting the enforceability of all remaining provisions. Pfizer reserves the right to alter or delete materials from this System at any time at its discretion.
